{"id":39474,"date":"2022-06-23T00:36:03","date_gmt":"2022-06-22T14:36:03","guid":{"rendered":"https:\/\/www.tomgrimshaw.com\/tomsblog\/?p=39474"},"modified":"2022-06-23T00:36:03","modified_gmt":"2022-06-22T14:36:03","slug":"94-percent-of-medication-not-supported-by-high-quality-evidence-harms-underreported-study","status":"publish","type":"post","link":"https:\/\/www.tomgrimshaw.com\/tomsblog\/?p=39474","title":{"rendered":"94 Percent of Medication Not Supported by High-Quality Evidence, Harms Underreported: Study"},"content":{"rendered":"<p><img loading=\"lazy\" decoding=\"async\" class=\" wp-image-39475\" src=\"http:\/\/www.tomgrimshaw.com\/tomsblog\/wp-content\/uploads\/2022\/06\/Syringe_In_Vial-300x180.jpg\" alt=\"Syringe In Vial\" width=\"603\" height=\"362\" srcset=\"https:\/\/www.tomgrimshaw.com\/tomsblog\/wp-content\/uploads\/2022\/06\/Syringe_In_Vial-300x180.jpg 300w, https:\/\/www.tomgrimshaw.com\/tomsblog\/wp-content\/uploads\/2022\/06\/Syringe_In_Vial.jpg 700w\" sizes=\"auto, (max-width: 603px) 100vw, 603px\" \/><\/p>\n<p>(Tom: I had wondered about this some time ago, for all the conditions for which medication is of assistance, is the average result from medication not detrimental.)<\/p>\n<p>Less than 6 percent of medical drugs have high-quality evidence to support their benefits, according to a recent study by the University of Oxford.<\/p>\n<p><a href=\"https:\/\/www.sciencedirect.com\/science\/article\/abs\/pii\/S0895435622001007#!\" target=\"_blank\" rel=\"noopener\">The study found<\/a>\u00a0that, of the 1,567 eligible medications approved under the Cochrane Reviews from 2008 to 2021, more than 94 percent were not supported by high-quality evidence.<\/p>\n<p>Cochrane Reviews is a leading international journal and database combining all available and relevant evidence about treatments and health care policies. It is often referenced in national and international health care guidelines, and is especially prominent in Europe.<\/p>\n<p>Reviewing Cochrane Reviews\u2019 35 percent of research papers, researchers found less than half of the drugs approved from 2008 to 2021 had moderate to high-quality evidence. Further, harms were underreported, with around 37 percent of interventions found with harm and 8.1 percent had significant evidence of harm.<\/p>\n<p>\u201cIt is particularly worrying that the harms of healthcare interventions are rarely quantified,\u201d wrote Dr. Jeremy Howick, one of the co-authors of the study, in\u00a0<a href=\"https:\/\/theconversation.com\/we-dont-know-whether-most-medical-treatments-work-and-we-know-even-less-about-whether-they-cause-harm-new-study-185167\" target=\"_blank\" rel=\"noopener\">The Conversation<\/a>.<\/p>\n<p>\u201cFor a doctor or patient to decide whether to use a treatment, they need to know whether the benefits outweigh the harms. If the harms are inadequately measured, an \u2018informed choice\u2019 is not possible.\u201d<\/p>\n<p>According to Howick, the \u201ccut-off\u201d year was 2008, as that was when Cochrane Reviews incorporated a system called grading quality of evidence and strength of recommendations (GRADE) to assess the quality of evidence.<\/p>\n<p><a href=\"https:\/\/bestpractice.bmj.com\/info\/us\/toolkit\/learn-ebm\/what-is-grade\/\" target=\"_blank\" rel=\"noopener\">The GRADE system<\/a>\u00a0used by Cochrane, is a transparent framework for developing and presenting summaries of evidence and provides a systematic approach for making clinical practice recommendations.<\/p>\n<p>It is a widely adopted tool for grading the quality of evidence and for making recommendations with over 100 organizations worldwide endorsing GRADE.<\/p>\n<p>Although authors of the Oxford study speculated that the poor findings for drugs backed up by high-quality evidence may be because the sample studies were \u201cunrepresentative\u201d of the population, Howicks argued that it was unlikely given the strictness of Cochrane Reviews.<\/p>\n<p>A 2020 U.S. study\u00a0<a href=\"https:\/\/jamanetwork.com\/journals\/jama\/article-abstract\/2758605\" target=\"_blank\" rel=\"noopener\">published in JAMA<\/a>\u00a0also indicated the problem of medications being approved without rigorous testing and causing harm. The study examined drugs approved by the Food and Drug Administration from 1983 to 2018 and found that whilst the number of novel biologics approved increased, the review period by the organization decreased.<\/p>\n<p>The authors wrote in the study that, the FDA \u201cincreasingly accepted less\u00a0data and more surrogate measures,\u201d clinical measures that show a correlation but may not necessarily guarantee a relationship.<\/p>\n<p>Dr. David Light from Harvard University argued that currently, 80 percent of drugs being prescribed that are known to work are still generic drugs, with experienced physicians recommending to not take new drugs for at least five years.<\/p>\n<p>Light wrote in his\u00a0<a href=\"https:\/\/ethics.harvard.edu\/blog\/new-prescription-drugs-major-health-risk-few-offsetting-advantages\" target=\"_blank\" rel=\"noopener\">2014 article<\/a>\u00a0that the majority of newly approved drugs bring little to no improvements, with 1 in 5 new prescriptions causing severe adverse events. At the same time, pharmaceutical prices for new drugs have soared with\u00a0<a href=\"https:\/\/www.theepochtimes.com\/median-drug-launch-price-85-times-higher-compared-to-2008_4520343.html\" target=\"_blank\" rel=\"noopener\">drug median release prices<\/a>\u00a0growing 85 times higher from 2008 to 2022.<\/p>\n<p>\u201cPotential harms should be measured with the same rigor as potential benefits,\u201d concluded Howick. \u201cThe evidence-based medicine community is correct to continue calling for higher-quality research, and also justified in their skepticism that high-quality evidence for medical treatments is common or even improving.\u201d<\/p>\n<p><a href=\"https:\/\/www.theepochtimes.com\/94-percent-of-medication-not-supported-by-high-quality-evidence-harms-underreported-study_4541149.html\">https:\/\/www.theepochtimes.com\/94-percent-of-medication-not-supported-by-high-quality-evidence-harms-underreported-study_4541149.html<\/a><\/p>\n","protected":false},"excerpt":{"rendered":"<p>(Tom: I had wondered about this some time ago, for all the conditions for which medication is of assistance, is the average result from medication not detrimental.)
